I am not sure when the same happened for the last time in Tests. Possibly in 1909-10. But it definitely happened in the exhibition game that followed the Lord's Test between England and South Africa in 1960.

Yes. The abovementioned Phil Gerrans. When Lancashire batted on for too long, Gerrans sent one underarm and was no balled for not notifying the umpire of the change of action.
