Cook went out of his way to get Pietersen back in he side, he's probably the last person that should be blamed for any ill that befell KP.
And I've never been aware of any long-standing dressing room issues re. Hales. AFAIK he went out of the side purely as a disciplinary response for the drugs thing, no other grievances bundled in there (although has MW1304 notes, the head-kicking thing is unlikely to have helped his cause). I don't think it should preclude him from ever playing again, but it's a perfectly legitimate reason to drop someone.
The England setup probably hasn't been exemplary in handling more difficult personalities generally, but I never like it (not saying you're doing this) when people take the stance that this gets said difficult personalities off the hook for what is sometimes genuinely dire behaviour.
