Over backing up, could be made a 5 run penalty for unsporting behavior from the umpire?
Umpire warning and then penalty, 3rd umpire could monitor.

No, it's way simpler if they just get run out.

Anything remotely unusual or controversial is going to be a bigger drama at lower level. You've got amateur umpires and often the batting team umpires their own side in lower level club cricket. Invariably leads to problems.
