When it is marginal, i.e half the balls width, the original decision stands every single time. I don't see how that is seen as an incorrect decision.Can't find a example at the moment.
But in a case the batsmen is given not out but the hawkeye says it is hitting the stumps ,then there have been cases where after calling it marginal the original decision has stood ,due to uncertainty.

I think that one demonstrates the need for UDRS just as badly. The umpire doesn't really have any choice but to give it not-out because it could have been missing, could have been outside the line and could have hit the bat, but with technology they can always get those ones right for the first time in cricketing history.
