Poor captaincy IMO. When you're defending a smallish target you're not going to get anywhere by protecting boundaries and giving up singles, you'll do it with wickets. Should have been a slip in.

Changed with the ICC revisions back in September-ish 2011, I think. At the same time that runners were banned for injured batsmen.

Last year along with the running between wickets/obstructing the field change iirc
